Cornwall Terrace Elementary School recently issued the following announcement.

2nd Grade Field Trip

When: April 26, 2022

Location: Reading Public Museum, 500 Museum Rd, Reading, PA 19611, USA

Description:

Parents/Guardians:

This is the tentative date of the 2nd grade class trip to the Reading Public Museum & Planetarium. While we make every effort to keep this date firm, there may be extenuating circumstances that cause us to reschedule.

Please be sure your child brings a packed lunch and wears the class t-shirt or other green t-shirt to the museum. Dress for the weather. Students will be walking between 2 buildings to see exhibits, the planetarium, and access to the bathrooms.

Please send your permission forms to school by Tuesday, April 19, 2022.  Teachers will send the permission forms home in March.  If you lose your permission form, please open and print the attachment.

Parents/Guardians interested in participating as a chaperone are required to have all of their clearances completed and submitted to the CT Office as soon as possible.  Some clearances may take 2-4 weeks to receive:

  • Act 34 - PA Criminal Background Check
    • Choose "volunteer" as the reason for the background check.
  • Act 151 - PA Child Abuse History Clearance
    • Choose "volunteer" as the reason for the background check.
  • FBI Criminal Clearance Form (Act 114)
    • Select Digital Fingerprinting
    • Use Service Code = 1Kg6XN
    • There is a cost associated with Fingerprinting of about $25.00
Questions about completing clearances?  Email Mrs. VanOstenbridge at vanpam@wilsonsd.org
